Applying for a Film Permit
The following information will be needed to apply for a Film Permit:
Company Information: Name and Address.
Insurance Policy: Insurance Company and Policy Number. Please visit the insurance information page for more information regarding where to send your insurance certificates and the appropriate information for additionally insured for the City of Miami Beach and Miami-Dade County.
Production Company & Production Contacts: Name, Title, Phone Number, Fax, Cellular, Pager, E-Mail.
Production Information: Production Title, Production Type, Extras, Cast, Crew, Dollars Spent Locally, Number of Production Days, Hotel Name, Location of Hotel, Number of Rooms per Night.
Location Request: Name, Address, Start and End Date, Description of Scene.

PortMiami location approvals must be obtained from the Office of the Port Director. A completed PortMiami Request to Film form and a completed POM Cast & Crew Security form (see PortMiami Filming Regulations for forms) must be emailed to Dorothea "Dee" Belz (email@example.com) Film and Entertainment Permit Coordinator, or Jeanie Lisenby (Lisenby@miamidade.gov), Permit Coordinator, Miami-Dade County Office of Film and Entertainment. At least one off-duty MDPD officer must accompany each production. The total number of MDPD and Port Security officers will be determined by the size, scope, and location of your project. Additional fees may apply.
